From One Knob to Four: Why Rat Wins Rarely Become Human Ones

Four-in-one weight-loss peptides are beating approved drugs in rat studies. Here's how we got from one hormone to four, where the human trials actually stand, and why a rat win is a rehearsal, not opening night.

From One Knob to Four: Why Rat Wins Rarely Become Human Ones

Your friend slides her phone across the brunch table. The headline says a new four-in-one weight-loss shot "crushed" the drug everyone's talking about. She wants to know if she should just wait for the new one.

Fair question. Here's a habit that's worth more than any headline: before you get excited, ask who was in the study. If the answer has whiskers, keep your expectations on a short leash.

How did we get from one knob to four?

Think of appetite as a stereo with a few knobs. The first wave of these drugs turned one knob, GLP-1, a gut hormone that tells your brain you've had enough.

Tirzepatide turned a second knob, a partner hormone called GIP, and it's approved. In its big human trial, people on the highest dose lost about a fifth of their body weight over roughly a year and a half.

Next in line are drugs that turn three knobs, or mix two drugs into one shot. CagriSema pairs semaglutide with an amylin-style "I'm full" signal. Retatrutide is one molecule aimed at GLP-1, GIP and glucagon. Both have gone through Phase 3, the big final round of human testing.

Neither is approved yet. Novo filed CagriSema with the FDA in December 2025 and says a decision is expected in the fourth quarter of 2026. When we checked on Oct 24, no decision had been announced.

Lilly has reported positive Phase 3 results for retatrutide and says it plans to file with the FDA in early 2027. Until a regulator says yes, it's still an experimental drug.

And four knobs? Nothing that hits four targets is approved anywhere. That's where the rats come in.

What are the four-knob candidates?

PTT-A, from a company called Pep2Tango, is a single peptide built to turn four knobs: GLP-1, GIP, amylin and calcitonin, the calcium hormone. One chain doing four jobs.

In two conference abstracts, obese rats got it for three weeks. At the higher test amount, they lost about 19% of their body weight compared with untreated rats, mostly fat, and the authors reported muscle was preserved.

The lower amount, tirzepatide and CagriSema each landed near 12% in the 2026 write-up. That's the kind of chart your friend saw.

Now the fine print. These are rats. Every author disclosed ties to Pep2Tango as an employee, shareholder, consultant or funded researcher. They're short conference abstracts, not full papers, and they don't publish the peptide's recipe, so outside labs can't check the molecule. We found no human trial.

Metsera's "M4" takes the opposite route: four separate long-acting peptides (GLP-1, GIP, glucagon and amylin) mixed in one shot, instead of one molecule doing everything. Company scientists reported rats losing more than a quarter of their body weight in under a month, more than with retatrutide.

Same caveats apply. It's animals, an abstract, and all three authors were Metsera employees. Even the abstract's title says mice while its text describes rats, which tells you how quickly these posters come together. Pfizer, which bought Metsera, is testing two of the four pieces together in people, and that trial hasn't posted results.

Why don't rat wins carry over to people?

Here's the myth: if it beat the champ in rats, it'll beat the champ in people. Here's the fix: rat studies are dress rehearsals. They're useful and necessary, but they're staged in a much smaller theater.

The proof is sitting right in this story. In the 2026 rat abstract, tirzepatide and CagriSema tied. When Novo ran the two head to head in people for about a year and a half, CagriSema fell short of tirzepatide and missed its own goal of showing it wasn't worse.

The rats also got very different amounts of each drug. That's one reason a rehearsal ranking can't set the final cast list.

Time matters too. Three weeks in a rat is a sprint. Human trials run longer than a year, with nausea, missed doses, holidays and real life, and a short study can't show whether the weight stays off or whether side effects pile up.

This isn't just an obesity problem. When researchers followed up on dozens of famous, highly cited animal studies, only about a third were later backed up by positive human randomized trials.

Literature, not lore.

So what's the future of GLP-1 drugs?

More knobs is a reasonable bet. Going from one hormone to two to three has held up in human trials, and amylin and glucagon keep looking promising. Whether four beats three in people, with fewer side effects and more muscle kept, is a question only human trials can answer.

For now, the real-world options are the approved ones, and that conversation belongs with a clinician. For the future, watch for the words "Phase 3" and "people," not "rats" and "poster."

A quick note on the four-knob compounds: PTT-A and M4 are investigational, aren't approved anywhere, and aren't for human consumption. Nothing here is a suggestion to use any compound.

If you remember one thing…

The future of weight-loss drugs is being rehearsed in rats, but it only opens in people. Until a four-knob peptide passes a human trial, that headline is a review of the rehearsal, not a ticket to the show.
