The 12% Belly-Fat Shot Measured One Slice of the Loaf
A shot that cut “dangerous organ fat by 12%” is all over your feed. The number is real. It also comes from about 40 people, one slice of an MRI, and a meeting abstract nobody has peer-reviewed yet.

The 12% Belly-Fat Shot Measured One Slice of the Loaf
Your friend at the bar pokes his own stomach and says, “Did you see the shot that zaps the fat around your organs? Twelve percent.” He’s already decided to wait for it instead of walking more.
Here’s the thing: that 12% is a real number from a real trial. It’s also a much smaller, narrower number than the headline makes it sound. Let’s slice it properly.
Literature, not lore.
Wait, what’s “organ fat”?
You have two main kinds of belly fat. Subcutaneous fat sits just under your skin. It’s the part you can pinch.
Visceral fat sits deeper, packed around your liver, gut, and other organs. You can’t pinch it, and it’s the kind doctors worry about. Big reviews link it to insulin resistance, type 2 diabetes, and heart disease more tightly than your weight alone.
The boring good news: you’re not stuck with it. A review of exercise trials found that regular aerobic exercise trims visceral fat in adults with overweight, even when the scale barely moves. Your friend’s walk still counts.
So what is this shot?
It’s called CBL-514, from a Taiwanese company called Caliway. It’s a small-molecule drug, not a peptide (we covered that in CBL-514 Is Not a Peptide).
It’s injected into belly fat. Caliway says it makes nearby fat cells switch themselves off, a tidy self-shutdown called apoptosis, without harming the tissue around them. That’s the company’s own mechanism story, mostly from its lab and animal work.
It was built to shrink the pinchable fat. Its earlier trials measured exactly that.
Where did the 12% come from?
From a short talk at Europe’s big diabetes meeting in Milan, Sept 28–Oct 2. A University of Alabama at Birmingham obesity doctor presented it with a Caliway scientist.
They went back to two earlier mid-stage trials of the shot. Then they pulled out only the people who happened to have MRI scans that could measure visceral fat. That left about 40 people, a bit over half of them on the drug.
Four weeks after their last round of shots, visceral fat on the drug was down about 12% from where it started. On placebo, it went up about 6%.
A month or two later, the drug group was still down about 10%. Placebo was up about 12%. That’s the number your feed should probably be quoting too, because it held up longer.
So why not just celebrate?
Because the 12% has three asterisks, and you deserve all three.
It’s a slice, not the whole loaf. The MRI measured visceral fat in one band of the belly, around the middle of your back, not every bit of organ fat in your body. Picture judging a whole loaf of bread by one slice. A good slice is encouraging. It isn’t the loaf.
It’s a small, picked group. About 40 people, chosen because their scans existed, not because the trial was designed to answer this question. The two groups weren’t perfectly matched either: the drug group was mostly women and started with a bit less visceral fat.
It isn’t peer-reviewed yet. This was a meeting abstract and a press release. No full paper has gone through outside reviewers. And the people presenting it work for the company or advise it.
The 12% is one slice of the loaf. Somebody still has to weigh the loaf.
One more detail the headlines skip: these weren’t people with obesity. Everyone was in the normal-to-overweight range. So this tells us nothing yet about the bigger bodies most weight-loss drugs are built for.
Does it help keep weight off after GLP-1s?
That’s the big pitch, and right now it lives in rats. In Caliway’s animal studies, rats given the shot plus a GLP-1 drug regained less weight after the GLP-1 stopped.
Even the UAB doctor said research is needed to prove that in people. A trial pairing the shot with Zepbound has been cleared to start, but it hasn’t reported anything.
So is it approved? When will we know?
No. It isn’t approved anywhere, and you can’t buy it.
Its first big Phase 3 trial, SUPREME-01, started in September with about 320 adults. Here’s the plot twist: its main goal is whether doctors and patients rate belly fat as visibly better, using a fat-rating scale. That’s the pinchable fat. Visceral fat isn’t one of the trial’s listed goals at all.
Results are expected in 2027. So the visceral-fat question may stay open even after Phase 3 reports.
The one thing to remember
If you remember one thing: the 12% belly-fat shot measured one slice of organ fat, in about 40 people, in an unreviewed abstract, and it’s still nowhere near a product. Meanwhile, the walk is already proven.
Inspired Labs doesn’t sell CBL-514, and nothing here is a dose, a protocol, or medical advice.
