Trevogrumab Isn't a Muscle Peptide. It's an Antibody in a Trial.
Forums call it a "muscle peptide" for people on GLP-1s. It's actually a big lab-grown antibody that has only been tested in a company trial, and it isn't approved anywhere.

Trevogrumab Isn't a Muscle Peptide. It's an Antibody in a Trial.
You're three months into a GLP-1 and the scale is finally cooperating. Then a friend at the gym squints at you and says, "You're losing muscle, too, you know. Get on the muscle peptide."
Half of that is true. The "peptide" part isn't. Let's sort it out.
Does losing weight on a GLP-1 really cost muscle?
Some, yes. When you lose weight by any route, part of what goes is lean mass: muscle, plus water and other non-fat tissue. In Regeneron's own trial, about a third of the weight people lost on semaglutide alone was lean mass.
The good news is that the fix with the most receipts is boring. A review of weight-loss research found that both cardio and resistance exercise help hold onto muscle while you lose weight, and resistance training also keeps you stronger. Enough protein helps too, though more isn't always better.
Think of your muscle as the furniture in a house you're downsizing. Weight loss is the moving truck. Lifting is how you tell the movers, "Not the couch."
So what is trevogrumab?
Trevogrumab is an experimental drug from Regeneron. It blocks myostatin, a signal your body uses as a brake on muscle growth. Take the brake off, and the idea is that the movers leave more furniture behind.
It's been tested in a Phase 2 trial called COURAGE, adding it to semaglutide in adults with obesity. One arm also added a second experimental antibody, garetosmab, which blocks a related signal called activin A.
What did the COURAGE trial find?
In the first part, about 600 people got semaglutide alone or with trevogrumab at one of two doses, or the three-drug combo. After six months, adding trevogrumab spared about half of the lean mass that semaglutide alone took away.
A second part tested lower doses for a full year. The better of the two still spared roughly two-fifths of the lean mass loss at 52 weeks. In an MRI substudy of thigh muscle, the company reported even bigger sparing.
The plot twist: at those lower doses, people didn't lose more weight. Trevogrumab changed what the weight was made of, not how much came off. The higher doses nudged fat loss a bit.
Now the grade, in one clause: this is one big, company-run randomized trial, reported so far through press releases and conference abstracts, with the full paper still "in press." Promising, not settled.
Was it safe?
Trevogrumab with semaglutide was described as generally well tolerated, mostly the stomach stuff you'd expect from a GLP-1, plus muscle spasms.
The three-drug combo was a different story. In interim data Regeneron released in June 2025, more than one in four people on it stopped because of side effects, compared with about one in twenty on semaglutide alone. Two people in that group died. One had multiple heart risk factors and an undetermined cause; the other had a cardiac arrest with a history of heart disease. Regeneron says it hasn't found that the treatment caused either death.
The company is now moving forward with trevogrumab, not the combo. The couch stays. The wrecking ball goes back in the garage.
Wait, why isn't it a peptide?
Because it's huge. Trevogrumab is a monoclonal antibody, the same family of drugs as many big-name prescription injectables. A typical antibody is built from four protein chains and weighs roughly a hundred times more than a 15-amino-acid peptide like BPC-157.
Peptides are short chains that a machine can build one amino acid at a time. Antibodies are grown by engineered living cells in big tanks, then purified. That's a different factory, a different rulebook and a different level of quality control.
So here's the myth: "It's a muscle peptide you can order." The fix: it's an investigational antibody that's only available inside clinical trials. A vial sold online as a "trevergumab peptide" isn't that drug, because the real drug isn't a peptide and isn't for sale. That label is a red flag, not a shortcut.
One more mix-up while we're here: trevogrumab is not guselkumab, sold as Tremfya, an approved antibody for conditions like psoriasis that works on a totally different target.
Literature, not lore.
If you remember one thing…
Trevogrumab is a lab-grown antibody, not a peptide, and it's still just a trial drug. If you're losing weight on a GLP-1, the muscle tools with real receipts are lifting, moving and eating enough protein. Talk with your clinician about that part.

Company topline, not peer-reviewed. Interim analysis when half of patients reached week 26: about 35% of semaglutide-induced weight loss was lean mass. Safety at data cutoff: treatment discontinuation for adverse events 4.6% with semaglutide alone vs 28.3% in the three-drug arm. Two deaths in the three-drug arm (one undetermined cause in a patient with multiple cardiovascular risk factors, one cardiac arrest with a history of cardiovascular disease); Regeneron has not identified a causal association.

Company topline of the complete 26-week results (about 600 participants in four arms): 33% of semaglutide weight loss was lean mass, and adding trevogrumab prevented about half of that loss. States that trevogrumab and garetosmab have not been evaluated by any regulatory authority.
